WAIVER AND RELEASE OF LIABILITY AGREEMENT
In consideration for being permitted to participate in boxing training, fitness activities, sparring sessions, and any use of facilities or equipment at The Lab Miami Boxing Gym ("The Lab"), I, the undersigned participant (or legal guardian if under 18), agree to the following:
1. Assumption of Risk
I acknowledge that boxing, sparring, and related fitness training are inherently dangerous activities that involve physical exertion and carry a risk of injury, including but not limited to cuts, bruises, sprains, strains, broken bones, concussions, or in rare cases, death.
I voluntarily assume all risks, known and unknown, associated with participation in these activities, including injury resulting from the actions or negligence of others, including trainers, staff, or fellow participants.
2. Release and Waiver
I hereby waive, release, and discharge The Lab Miami Boxing Gym, its owners, employees, agents, volunteers, and any affiliated parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) that may be sustained by me while participating in any gym-related activities or while on gym premises, including travel to and from the facility.
This release includes, but is not limited to, liability arising from negligence, whether passive or active, and assumes full responsibility for any injury or loss.
3. Medical Clearance & Responsibility
I represent and warrant that I am in good physical condition and have no medical condition that would prevent my safe participation in boxing, fitness training, or sparring. I acknowledge that The Lab Miami Boxing Gym has strongly recommended that I obtain a physician’s approval prior to beginning any exercise or sparring activity.
In the event of an emergency, I authorize The Lab staff to obtain medical treatment for me, and I agree to be financially responsible for any medical care needed.
4. Sparring Disclaimer (If Applicable)
I understand that participation in sparring carries a heightened risk of injury. Protective gear (headgear, gloves, mouthpiece, groin protector, etc.) is required and must be worn at all times during sparring.
I voluntarily agree to sparring activities, understanding that no amount of safety equipment or supervision can eliminate the inherent risk of contact sports.
5. Rules and Conduct
I agree to follow all posted rules and staff instructions at all times. I understand that failure to follow rules or disrespecting staff or other members may result in termination of my membership without refund.
I acknowledge that no drugs, alcohol, or unsafe behavior will be tolerated on gym premises.
6. Photo/Video Release
I grant permission to The Lab Miami Boxing Gym to use photos or videos taken of me during training, events, or classes for promotional purposes, including on social media, unless I provide written notice opting out.
7. Minor Waiver (If Participant is Under 18)
As parent or legal guardian of the minor named below, I consent to their participation and agree to the terms above on their behalf. I affirm that I have the legal authority to sign this release.
8. Acknowledgment of Understanding
I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY AGREEMENT. I FULLY UNDERSTAND ITS TERMS AND UNDERSTAND THAT I AM GIVING UP LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. I SIGN IT FREELY AND VOLUNTARILY.
